I realize that the problem could be that community-organized software design doesn't work in practice.

If that is the case we should be talking about it, not just pretend we're cool and talk about open source.

Google and Facebook agreeing behind closed doors will result in something that works for major corps. That doesn't mean it works for us.

That being said, Yarn seems to solve some real issues, and npm certainly isn't perfect.
